Login to Mitel Connect
When you first launch Mitel Connect, it will ask you for "Email or username" and Password. Below this is a link that says "Show Advanced".

Click on Show Advanced.

Right below "Show Advanced" there is a checkbox for "Use Windows Credentials". Check the box, uncheck it, then check it again and all of the required fields should fill in.
If for some reason the Server does not fill in, most users can enter "Shoretel-HQ.anc.local". ISSB users will need to enter "issb-dc1-mtl-01.anc.local".
